This form is provided for easy ordering and is for etchings on "Absolute Black" granite.  For etchings on other surfaces such as red granite, marble, glass, mirror, plastic, metals, ceramics or acrylics, or for special projects such as backsplashes, murals,  cornerstones, table and countertops, trophies/awards,  please send an e-mail to John Arends or call us at 616-669-4093 for price quotes.

This agreement authorizes Loving Etches to proceed with my order as described above.  I understand that due to granite being a natural product,  end results may vary.*

To order:

1.  Select a photograph (or artwork) of the image you want etched. (Note: Images may be in color but will not appear in color. Make sure the picture is in focus. All pictures will be re-touched on the computer to get the best possible contrast and etching possible.  If you wish for only part of the picture to be used please indicate so on this form.  It is recommended you protect pictures between cardboard. We can not be held responsible for any damages during shipping.  Your photo remains your property and will be returned as received.) Images may also be sent to us via e-mail as an attachment, please make sure you include your full name.

2.   Print this form  and fill out completely, and sign.

3.  Complete check or money order made payable to Loving Etches. (Name and address on the check must match the name and address to which this order will be shipped.  Checks must clear the bank prior to your order being shipped.)
